Initial Comments:An offsite follow-up survey conducted on December 7, 2023, found that At Home Rehab LLC had corrected the deficiencies cited under the requirements of 42 CFR, Part 484, Subparts B and C, Conditions of Participation: Home Health Agencies. The deficiencies were cited as a result of a Medicare Re-Certification Survey completed on October 26, 2023.

Code, Part IV, Health facilities, Subpart G. Chapter 601. The deficiencies were cited as a result of a State Re-Licensure survey completed on October 26, 2023.
